Circle of Care Collaboration Delivers Successful Vaccination Clinics Across the Community

"RSV Clinic Staff and Volunteers"At Circle of Care, we believe that exceptional care happens when providers come together to meet people where they are: right in their homes and communities.

In the last month, this belief was powerfully demonstrated through a successful partnership with Ontario Health and Central Ontario Health atHome to deliver Respiratory Syncytial Virus (RSV) vaccinations, along with COVID and flu vaccines, to seniors in the GTA communities.

"Two seniors sitting with nurse after vaccination"Over the course of four clinics held at three different locations — including our Adult Day Program (ADP), Bernard Betel Centre, and an assisted living facility — Circle of Care brought vital preventative healthcare directly to the 83 people who are most vulnerable to these infections in familiar, supportive settings.

Leveraging the pre-existing relationships developed over years of community programming partnership, Circle of Care handled clinic logistics, set-up, awareness campaigns, and registration. This ensured a smooth, well-coordinated process for our clients to receive their vaccines administered by registered nurses Central Ontario Health atHome.

Our Adult Day Program provides a safe and engaging environment for seniors who are frail or living with dementia, offering social, recreational, and health support during the day. Hosting the vaccination clinic at this location was especially meaningful, as participants were able to receive their vaccines in a familiar setting, surrounded by staff they spend time with throughout the week and have come to know and trust. The families of two clients shared that they were surprised their loved ones successfully received the vaccine, as they had anticipated potential challenges.

"Photo of RSV Clinic"Similarly, clinics at the Bernard Betel Centre and the assisted living facility demonstrated our commitment to bringing care directly to seniors — right where they live and routinely socialize — making access easier, more convenient, and seamlessly integrated into their daily routines. One client shared that due to snowy conditions, they were unable to get a vaccine earlier and were particularly grateful the clinic was brought right to their doorstep.

The effectiveness of these clinics was not only reflected in the verbal appreciation we heard from clients, but also clearly demonstrated in the results of a survey we did with the participants. An overwhelming 97% agreed or strongly agreed that the clinic location was convenient. Nearly all respondents (99%) found the registration process, information provided, and directions to the clinic helpful. Similarly, 99% recognized the care and skill of the nurses during vaccine administration, and 99% agreed or strongly agreed that the staff and volunteer team helped address their comfort and mobility needs. The survey was also offered in Russian at the location where it was most relevant, helping to ensure accessibility and inclusive participation.

"Two RSV Clinic Staff reviewing documents"The collaborative approach in this initiative not only improved vaccine uptake but also strengthened connections between home and community care providers and health system partners, reinforcing a more coordinated and accessible network of support for seniors, especially those most vulnerable and frail.

Circle of Care is proud to have led this effort and looks forward to continuing to foster partnerships that make it easier for seniors to get the care they need in a way that is accessible and comfortable.
