Pre-Authorize Your Payment for Services

Please complete the following steps if you wish to set up pre-authorized payments for your Circle of Care services:

1. Complete the form below, and then email it to the Finance Department: financedept@circleofcare.com.
2. Phone the Finance department to provide us with your credit card information that we can input directly into our payment system: 416-635-2860 ext. 270.
