What the Role Looks Like

As an Occupational Therapist, you will:

  • Conduct in-home assessments to identify barriers to daily living.

  • Recommend and fit assistive devices to promote safety and independence.

  • Support patients in achieving SMART goals for recovery and self-management.

  • Provide education and guidance to families and caregivers.

  • Collaborate with the care team to ensure smooth transitions from hospital to home.

This is a community-based position requiring travel to client homes across Toronto.

What You Bring

We’re looking for Occupational Therapists who are compassionate, collaborative, and committed to patient-centered care.

Qualifications:

  • Occupational Therapist with current registration with COTO.
  • Experience working as part of an integrated team with knowledge of regulated and unregulated health care providers, including rehabilitation assistants.
  • Experience working in the home care sector.
  • Current ADP authorizer for Mobility or eligible to apply.
  • Commitment to evidence-informed clinical practice and outcome measures; professional practice models and practice requirements in Ontario.
  • Commitment to person-centred care and cultural diversity with knowledge of population health and vulnerable populations e.g. elder adults, Indigenous people, people living with mental health and addictions, 2SLLGBTQI+
  • Experienced supporting patients with Acquired Brain Injury or post stroke recovery.
  • Excellent knowledge of the social determinants of health, impact on clinical outcomes.
  • Excellent communication, collaboration, problem solving, and assessment skills.
  • Ability to work in a fast-paced setting, highly organized, and ability to work independently and function as a member of a team.
  • Additional language skills are an asset, preference for French speaking.
  • Computer literacy skills and compliant with PHI cybersecurity guidelines.
  • Valid Driver’s license and vehicle required.
