An Innovative Approach to Dementia Care
For seniors and adults facing physical and cognitive challenges, home care services typically focus on supporting physical health and daily living. However, mental health and social needs are just as vital for overall well-being. Engaging in community programs, small group activities, and socializing in supportive environments help combat loneliness and isolation, especially for individuals with dementia or physical frailties. These activities not only enhance quality of life of the participants but also provide their caregivers with much-needed respite, strengthening the support system around the individual.
Circle of Care’s Adult Day Program (ADP) offers daily enrichment activities that keep participants active, socially connected, and mentally engaged. This approach helps individuals continue living independently at home, benefiting from both social and physical stimulation in a safe community setting. It also ensures that caregivers can rest easy, knowing their loved ones are well-supported, engaged, and thriving.
What Can I Expect from Circle of Care’s ADP?
Our wheelchair-accessible facility offers a diverse range of programming seven days a week, led by experienced professionals such as social workers, therapeutic recreationists, and program assistants. Designed to support adults living with dementia, Alzheimer’s disease, memory loss, and physical frailties, the program emphasizes social connection, physical activity, and cognitive stimulation.
Here’s what we offer:
- Social Activities: Shared interests like baking, gardening, and small group activities help clients connect and build community, while providing therapeutic, occupational, and cognitive benefits.
- Physical Activities: From gentle chair exercises and Tai Chi to our innovative virtual cycling program Centivizer – 2RaceWithMe, our therapeutic recreationists tailor activities to participants’ abilities, helping clients get moving safely.
- Creative and Cognitive Stimulation: Programs include music, trivia, art therapy, and innovative tools like Rendever Virtual Reality Therapy and our interactive Lucynt table. These activities engage the senses and help enhance memory and cognition.
In addition , we offer free hearing clinics and a range of spa services including hairdressing, Reiki, and foot care for an additional fee. These services provide moments of relaxation and pampering for clients and caregivers alike.
Practical Supports
Transportation to and from the program is available for clients living within our boundaries (South of Major Mackenzie, North of Finch, West of Bayview, and East of Dufferin), though participants from outside the area are welcome to attend the ADP. Participants also receive hot meals and snacks, medication administration, and assistance with health care needs. In addition to much needed respite for caregivers, we also provide counselling, education and support for program participants and caregivers.
Join “The Club”!
Circle of Care’s ADP is much more than just medical care. It’s a community where participants find joy, caregivers find respite, and everyone benefits from a supportive community of care. Families often call us a “lifeline” in their caregiving journey, while clients affectionally refer to our program as “The Club.”
How to Access the ADP
To learn more, call us at 905-728-1128 to speak with a social worker who can guide you through the referral process with Ontario Health at Home (OHaH) and explore funding options. An in-home and on-site assessment with an OHaH care coordinator will help ensure the program will meet your needs. If you are eligible to attend, the care coordinator will assist you with finding the programs and schedule that meet your needs. If you are not eligible, they will help you explore other alternatives.
Program Fees
Circle of Care’s ADP is partially funded by the Ministry of Health, with additional fees covering activities, meals, and snacks. Depending on your circumstances, you may be eligible for subsidies to help offset costs.
We’re excited to welcome you to our program and support you on this journey!
