The health care landscape is undergoing a period of significant change and renewal. A key component of this transformation is the creation of the new Ontario Health Teams (OHTs), which are part of a new model of organizing and delivering services in local communities. Circle of Care, along with several health care partners, was just approved to move forward to the next stage of planning for the development of a North York Central Ontario Health Team.
Under these teams, health-care providers will work together to provide integrated health care for a defined patient population within a specific geographic region. These teams will be responsible for coordinating and overseeing the delivering of all care for their patients, including understanding their health care history and directly connecting them to the different types of care programs and service providers they need.
While the goal of these teams is to care for entire populations, the North York Central OHT will be starting with certain groups in our community who have more complex and higher needs, including frail seniors, those with mental health and addiction issues, and those at end of life.
